We use the information entered into this form only for the purposes of processing your Friends & Family Test submission. Information entered into this form is stored and accessed securely by designated Practice staff.
Issues raised in comments may be discussed between relevant members of the Practice. The information is used for quality monitoring purposes, in line with the expectations of the patients submitting the feedback.
The form does not require personal information. Any personal information transmitted via this form may be anonymised by the Practice when this is required to ensure compliance with General Data Protection Regulation.
All submissions are collated and sent to NHS England.

New appointment system starting 27th February 2024.
We are making changes to our appointment system in line with a contractual requirement from NHS England. This change aims to improve access to medical services for all our patients through digital consultations.
How will the new system work?
To request an appointment with a clinician, you will need to complete a medical form. This form provides details about your health concern, allowing us to:

Make your consultation more effective, as the clinician will already understand your concern.

Give you the time and space to explain your issue fully.
What are the benefits?

No more queuing outside.

Appointments will be allocated based on need, not on a first-come, first-served basis.

Instead of waiting for a convenient day off to visit, you can submit your form as soon as an issue arises. We will arrange an appointment – whether same-day, in a week, or up to six weeks later – depending on clinical need. Appointments may be face-to-face, over the phone, or via video consultation, as appropriate.
Can’t I just book an appointment?
No, this will no longer be available. You will need to complete a medical form prior to an appointment being booked with a clinician, with the exception of Practice Nurse appointments.
What if I need urgent treatment?
No, this will no longer be available. You will need to complete a medical form prior to an appointment being booked with a clinician, with the exception of Practice Nurse appointments.
Do I need to complete a form for regular nurse appointments?
No. Nursing appointments can still be booked directly by calling our reception team.

Are You a Carer?
Are you a carer or cared for? Do you care for someone who could not manage without you or are you someone that is cared for?

It is important that carers stay as fit and healthy as possible. Please register yourself as a carer here.

Under NHS regulations all GPs are required to compile a register of carers. This identifies carers and their needs so that support can be offered when needed if you care for a relative, friend or neighbour who is dependent on that help. The person being cared for may be handicapped, have a debilitating physical or mental illness or be in frail health and can be any age.
